For crew members who will embark on CIA Seabourn and Holland AmericaN, please open your request by selecting the Company "Certificado Norueguês (ENG1)". It is worth emphasizing that the Norwegian certificate is only issued by our doctor in São Paulo city, so the crew member must undergo this procedure in this city.
For CIA Harding Retail, please check with the company which form will be used. If the NCL form is used, we request that the occurrence be opened as Norwegian Cruises Line.
For crew members who will take the exams with Princess, please open the registration with the CIA "Norwegian certificate (ENG1) CIA Princess and the issuance of this certificate with our doctor is only carried out in the city of São Paulo. If you wish, you can take the exams in our partner clinics and when the results are ready, you can schedule the certificate to be issued with the Doctor and leave with it on the day of your appointment.

I am aware and authorize Sampletrip - a company of the Samplemed Group - to conduct the exams that are within the profile of the airline to which I will apply. These may include X-rays, audiometry, electrocardiograms, stress tests, and laboratory analyses, which, in addition to conventional analyses, may include HIV testing (performed during blood collection) and toxicological exams (performed during urine collection). I am also aware that the information reported in the health form, as well as the results of all exams, may be submitted to the airlines, as some already require this to issue the medical certificate or will be sent to those that request it.
I am aware that to issue the certificate it will be necessary to visit the city of São Paulo.
